A Case Study of Family Therapy on Factors Affecting a Korean Child with Tic Disorder and Attention Deficit Hyperactivity Disorder

This study explored the factors influencing tic disorder and Attention-Deficit/Hyperactivity Disorder (ADHD) of a Korean child using the Korean Integrative Family Therapy Model. Primary coding was conducted to derive key concepts, followed by secondary pattern coding to illustrate the relationships between categories using network analysis. Three triggering and underlying factors affecting symptom occurrence, maintenance, and exacerbation were identified. Triggering factors include witnessing a child's traffic accident, witnessing conflict between father and paternal grandfather, and emotional outbursts by the mother, all of which act as triggers for tic disorder and ADHD in the child. Underlying factors influencing symptoms include paternal grandfathers' patriarchal family culture, transference, and dysfunctional family communication patterns. Specifically, a patriarchal family culture includes chronic anxiety caused by the father, strict family rules, and the father's inability to mediate conflict. Transference involves the transference patterns of both parents. Dysfunctional family communication included ineffective and dysfunctional communication between parents and indirect communication between maternal and paternal grandmothers. These findings suggest that family therapists counseling Korean children with tic disorder and ADHD need to focus not only on the child's symptoms but also on Korea's patriarchal culture, parental differentiation within the extended family, transference, and communication patterns underlying the symptoms.
